NDIS Functional Capacity Assessment & Support Needs Recommendations
Comprehensive assessment, recommendations and report writing to outline the reasonable and necessary supports required for you to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in your community, and improved quality of life.
Inclusions:
- 16 hours of combined face-to-face assessment and non-face-to-face assessment scoring, interpretation, support planning and report writing.
Optional Extras:
- Access Request & Change of Situation consultation and/or coordination. Charged at standard hourly rate.

We use a combination of approaches including education, skill building, physical and cognitive exercise, environmental modification, activity adaptation, use of assistive equipment and advocacy. Complex Multidisciplinary Handover/Referral*
Handover between clinicians of detailed accurate and relevant information can be extremely helpful in reducing your energy expenditure when engaging new practitioners or specialists to complete assessments, treatment or legal documentation in relation to your complex or interacting conditions. Not only does it save you energy but it can also prevent continuously reactivating trauma through retelling and reliving your story.

- ~ 2hrs of non-face-to-face detailed written clinical handover
